Documents for obtaining a commercial identity certificate
Commercial Identity Certificates are issued only to natural persons. In order to issue commercial identity certificates, the following documents must be presented when visiting the registration authority. For more detailed information on the required documents, please refer to the Certification Policy for the Electronic Identification System.
Power of Attorney - acting on behalf of the CertificationPolicy
Non-entrepreneurial natural person
- ID card** or passport or similar document of equal legal weight
- other proof of identity (health insurance card, passport, driving licence...)
Acting on behalf of
If the applicant for a certificate is represented at the I.CA by an agent, the above documents must be accompanied by:
- ID card or passport of the agent
- other proof of the agent's identity
- a document proving the right to act as proxy; model above - the signature of the "Proxy" on the power of attorney must be officially certified
- original or certified copies of the applicant's ID card** or passport and other proof of identity of the applicant for the certificate (principal); please note that certified copies of personal documents cannot be made in the Czech Republic
- other documents according to the type of certificate, see above
** University diploma - the mandatory application item "Full name of applicant" may include the applicant's full name including degrees as it appears on his/her primary identity document. If the title is not listed on the primary personal document, a diploma must still be provided stating that the applicant has the right to use the title.
