Documents for obtaining a qualified certificate for the eSeal
In order to issue a qualified certificate for a guaranteed electronic seal, the following documents must be presented when visiting the registration authority. Legal entity
- Identity card or passport or similar document of equal legal weight
- other proof of identity (health insurance card, passport, driving licence...)
- Proof of existence of the company* (original or certified copy of an extract from the commercial or other statutory register/register, trade certificate, memorandum of association or other document of equal legal weight, which must include the full business name, identification number (if assigned), registered office address, names of the person(s) authorised to represent the company (statutory representatives) and the manner in which they act and sign on behalf of the legal person)
- a document authorising the person to act for the company - power of attorney (officially certified powers of attorney are accepted), letter of appointment of the managing director, or both); specimen above
Acting on behalf
If the applicant for a certificate is represented by an agent at the I.CA registration authority, the above documents must be accompanied by:
- ID card or passport of the proxy
- a document proving the right to act as proxy; model above - the signature of the "Proxy" on the power of attorney must be officially certified
- original or officially certified copies of the ID card or passport and other proof of identity of the applicant for the certificate (principal); please note that certified copies of personal documents cannot be made in the Czech Republic
- other documents according to the type of certificate, see above
* The document must not be older than six months; in the case of entities registered in the Commercial Register of the Czech Republic or in the Trade Register, it is not necessary to submit this document (the document is available in electronic form on the website of the Ministry of Justice or the Ministry of Industry and Trade and can be obtained directly from the I.CA registration authority).
