In order for the registration authority to be able to issue of qualified certificates for eSeal for creating of electronic seals it is necessary to submit the documents indicated below. For more information on the required documents can be found in the Certification policy for qualified certificates SK area.

Legal entity / public authority

identity card or passport, respectively a similar document of the same legal weight,
other identity document (health insurance card, passport, driving license ...),
substantiating the company * (original or certified copy of the business or other legally designated register, trade license, foundation deed respectively another document of the same legal weight and which must include full business name, identification number (if assigned), registered address, name of the person or persons authorized to represent the (legal representatives and the way for a legal person act and sign)
a document authorizing negotiations for the company - power of attorney (accepted are officially certified power of attorney, appointment letter of directors, or both); template power of attorney on behalf of Negotiations.

If an applicant for a certificate is represented by an agent during the negotiations with the I.CA registration authority, the above documents must be accompanied by:

the agent’s national identity card
another identity document of the agent
proof of the right to act as an agent; sample power of attorney - The signatures „Principal’s” on the Power of Attorney must be authenticated
originals or officially certified copies of the identity card or passport and a second identification document certificate applicant (the principal); Please note that the Czech Republic can make certified copies of personal documents
other documents according to the type of the certificate (see above)

* This document may not be older than six months; in the case of entities registered in the Commercial Register of the Czech Republic or in the Register of small businesses, it is not necessary that document submitted (document is available in electronic form on the website of the Ministry of Justice, respectively. Ministry of Industry and Trade, and can be taken directly to the registration authority I.CA).

** A university degree - obligatory item request "Full name of the applicant" may contain the full name of the applicant, including titles, as stated in his primary personal identification document. If the title is not in the primary personal identification document presented, it is still necessary to prove a diploma, stating that the applicant has the right to use that title.
